Recipe ingredients could not be organized into groups or reordered, making complex recipes harder to follow.

This adds ingredient groups, ordering controls, validation, persistence, and updated recipe/MCP support.

Closes #239

Make the e2e drag step actually drag and assert the persisted order, move grouping coverage to groupIngredients, and table same-shape cases.
Renaming the migration regenerated its journal timestamp, so databases that
already applied it tried to run it again and failed.
- derive editor groups from props and rely on server-side query refresh
- share ingredient group insertion and relation ordering between services
- reuse ingredientNameGroups and listProposalIsStale for assistant staleness
- consolidate ingredient group helpers and types
- restore optimistic ingredient edit and delete
